Default The Executor has died.....now what?

If Henry dies and John is the executor of Henry's will and then John dies without having fulfilled the obligations of being the executor, what happens next? Does the wife of John become the executor? There are grown children of Henry.

The court will often consider who can take over and make an appointment, unless the will covers the issue already.
