My father just passed away a couple of months ago I am his eldest daughter and we are trying to file a claim for benefits from his previous employer and they told me I need a 'Power of Attorney' or an 'order to dispose form' in order for them to release his medical records in order to file the claim. my father's brother acted as a power of attorney when my father had to get surgery is my uncle still considered power of attorney? how do I go about getting this 'Order to dispose form' in the state of ga. in order to obtain my Fathers medical records? I have my birth certificate.

You will have to go to Probate Court with a copy of your father's death certificate and file to be the Appointee of his Estate. If he died intestate, you need to be named as the person responsible for overseeing his Estate.
