My sister was applying to lease an apartment. Because she was a student and unemployed, she could not fill out the credit application (background forms for application), so I filled one out with my name and income information. The lease company said she had to have a guarantor. She was approved to lease the apartment, and signed the lease. I am not on the lease. If she were to terminate the lease, would I have any responsiblities financially because of the initial credit forms that I filled out? Again, I am not on the lease. Thank you!

Absolutely. If you are the guarantor of her lease, you are the financially reponsible party if she defaults on it.
