Default Is FMLA my coice

I am having an out patient surgery that will require me to be out for 3-5 days. I had planned on taking pto for this prcedure, because the doctor told me I could go back sooner if I felt ok.
My Employer is making me take FMLA ,that I am not requesting. I was told I HAD to take it by my employer even after I told them I didn't want it that I only wanted to use my PTO ( vacation time). and that i couldn't come back until I had a doctors release. My doctor is charging $15 per page to fill out there are 6 pages for my doctor to fill out. is any of this nonsense legal . I feel that I am being screwed by both the employer and doctor .
any input would be appreciated
