Unhappy Money Hungry Doctor

I recently underwent a surgery and was off work for 3 weeks. I wanted to
file a disability claim with my insurance company but my doctor refuses to
fill out the necessary form due to the fact that I owe them some money (co pay) Is
this legal?

No. You could report him to the medical licensing board. You may want to let him know that.
