work mans comp issue for legal advice

i need ur advice on workmans comp .. here is situation .... last yr sprained ankle at work-- work would only let me go to urgent care- urgent care took side views of ankle - didnt see ankle was broken in front now having lots of problem- went to my own doctor ankle specialist - he said it was broken in the front and i cant walk on it needs surgery-- company changed workmans comp companys and cannot find paperwork on it and where they might have filed it---- please give me some advice to who to talk to or what should i do , i am out money, i cant work , and now im losing my insurance because it was based on hours worked.....
