Default Immigrant Visa

Do I need an immigrant visa to adjust my status to that of lawful permanent resident or will an approved I-130/immigrant visa number suffice? Can I adjust my status from within my country and then travel to the us as a lawful permanent resident? In case I’m not eligible for an immigrant visa.
