how do you know when a lawyer should be closing your case

I have accepted a settlement in my case about a year or more, have signed the agreement. The judge has approved the settlement or should I say the last time I spoke tothe lawyers/ paralegal they were waiting on medicare keep in my this settlement was accepted last year.It appears to be one thing after another when it come down to the lawyer giving me the settlement.The judge had the settlement placed in escrow this was sometime last year until the legal heirs issue was settled the last time I went to court we satisfied this issue now I am informed that there is another problem, my question is who do I go TO TO ENSURE THAT THE LAWYERS ARE STICKING IT TO ME, the bar association.
