Wink Using the Father's Surname for the Unwed

Good day!

If you could provide me a sample copy of the Affidavit of Using the Surname of a Father since the parents were not married.

It is very much appreciated if you could help me one.. for free of course...

Why would you need an Affidavit of using the father's surname?

What is it that you are trying to prove?

Don't you have any affidavits from prior motions in your case that you can use as a template?
