Default multiple powers of attorney

Myself, along with my 2 sisters, have power of attorney for healthcare and financial matters for our elderly mother. She is now at the point of wanting to go into a long term care home. Our mother is 87yrs. old and would like us to take care of it for her. When we need to give the go ahead for any health care, or, if we need to get money from her bank accounts, do all 3 of us need to sign for these things? I certainly hope that we wouldn't, especially since one is on the west coast, while I am on the east!!
Also, is the paper from the lawyer stating that we are her powers of attorney, enough to verify this to the bank or health care provider?

if all three of you are listed as power of attorney then all 3 have to sign,unless you get a notarized document from the other 2 saying you can sign for everything. If one is primary and the others as alternate then the primary signs for everything. Each one of you can give the other the authority to sign if you get the notarized document stating it is ok but it should be specific to what and how long.
