Default Recovery of an IRA: Will a Quadro Form sufice?

My wife and I were divorced last year. There is a Roth IRA that she has agreed to turn back over to me as stipulated in the property division.
The IRA is in her credit union. I have been able to have mine transfered to my bank. If she continues to procrastinate, What avenues do I have to recover the one she has agreed to turn over?
Will a Quadro Form sufice?
