Exclamation Employer opened credit card under my ssn?

Could someone give me advice on what steps to take against my previous employer? The employer/owner of the company opened a business credit card under my social security number. I have been gone from the company for over a year and a half but recently I received a call from the credit card company telling me that I have an unpaid bill with them and that it was opened back when I was employed there. I had been a card user along with 4 other employees but after further research they told me that I was the guarantor on the account. The owner of the company receives the bills in my name but the companys address which would tell me that the employer/owner knows of the fraudulent account he is using. I know that I can have the employer sign a document taking the account out of my name but my credit score has been adversely affected for the last year and a half due to the employer paying the account over 30 days every month. Your advice would be greatly appreciated.

You may file a police report for fraud and forgery; and sue in small claims for damages and fees etc. for credit monitoring, other damages.
