Confused Filing an appeal

Is there any information as to the procedures and forms to file an appeal? I recently found out that a previous ruling of the courts was based upon incorrect information that was written by a hearing officer and signed by a judge. The information supplied in the report and recommendation by the hearing officer has several items that are false and incorrect.

You normally will want an appeals lawyer to prepare it.

Or at least law students or legal aid. Appeals can be technical.
