Is this civil or criminal.......

I have discharge papers from a hospital. I had complained and told them I was looking into hiring an attorney.

I received medical records today. I left against medical orders. On my discharge sheet I can tell the signature is mine. Then there is a sheet that says I cannot sue the hospital,doctors, etc. they are a place for a witness to sign and it is blank. The most important part is the signature is NOT MINE. I have had the same last name for 25 years. They even forgot to add the "e" in my last name.

What can I do about this?

You can file a police report for forgery. That is illegal!!

I live in MO...this happened in IL

Do I file it in my home town or in the county the hospital is in?

It is the Holiday's and I haven't been able to find out anything.

Wherever the act act occurred but report it first to your own police and they will direct you.
