Confused Civil Court Papers

The man refered to us and hired to redo the pool took the money started but never completed the job. what papers are needed to file a civil claim? Thank you in advance for any and all help.

Confused

Thank you very much but It is more than the $2000 that Mass allows for small claims court. I was told that I would have to go to civil court to get the money back, but I don't know which forms I need to file the case. I'm not sure if it's larceny, fraud or what.
