A revised parenting agreement submitted to the courts. Beyond that...I don't know. There is an ask an attorney section for 10 dollars or you can try to find a paralegal service that may help.

I don't know the form for MN, but in CA its a Notice of Motion (then check off modifcation/custody). If you went to mediation though, that would be the way to modify since it would be an agreement between both parents rather than a judge making the determination on which side to believe.
