How do i ask for a motion to quash medical?

I have been informed by my employer that I will have to provide medical coverage for my daughter. My daughter has coverage through her father and a Federal Medical Program.(I.E.H.P.) I provided the Family Support Office with a copy of her Medical Card, and was told that it was not sufficient coverage. I explained that I could not afford the cost of coverage through my employer. There was no reasonable cost available. I was then told that I would have to file a motion to quash the request for medical coverage. I asked how to go about this, they told me to contact the court. I called the court to ask which forms to use, they told me that I would have to come in, no info could be given over the phone. I explained that I live in a different county and it would take over an hour to drive there, that I did not have sick leave accrued was there anyone that could talk to me. Of course my answer was no. I just want to know which forms I need,(downloaded all the family law forms available) and what is the step I need to take. My employer keeps asking me what I have decided to do about the coverage. I can't afford the additional $300 a month on top of the child support I already pay. Is there someone who will talk to me?

First, what agency directed your employer to tell you that you need to provide medical coverage?

Second, your court orders should specify which parent is responsible for maintaining medical coverage. Read them carefully, and make sure that the agency is interpreting them correctly.

Third, there is no "form" that you fill out with the court. You type up a motion requesting the modification you seek. I assume that there have been motions filed with the court on your case before, and you should have copies of them that you can use as templates to write your own. Along with the motion, you may want to file an affidavit asserting the facts you know to be true, and you will sign this affidavit before a Notary Public as a sworn legal document. Your bank will have a notary public on staff.
