Just go back to court and tell the judge what has happened and where your son is living. The order will be changed.

You may write the judge and ask for a hearing if you don't have a lawyer involved.

File a motion with the court asking for custody of your son. Ask for an order of temporary custody until the hearing. With the motion, file an affidavit stating the circumstances, along with any evidence or documentation you have.
