Confused Parent walks away from child

Can anyone tell me in New York State what constitutes child abandonment? What factors are used to decide this? What does one need to do to pursue having a persons parental rights taken away or at the very least visitation stopped?

I am the childs grandmother. I have primary residential and physical custody of my grandson. There is joint custody between myself, my daughter and the childs father. My daughter and I are looking for the information.

The exact wording was as I stated, we share custody with my grandson living with me, giving me primary physical and residential custody. The father was to have visits 3 days a week for 1 hour per visit until furture notice. He has failed to do this. My daughter has visits anytime she wants them, she is away at school so she comes home every other week-end, all holidays and vacation times.

...does not sound like any legal language I've ever seen in any court order.
