terminate access?

i have full sole custody of my 7 year old daughter i have had full sole custody
for almost 8 years. The mother of my daugter has been involved with the C.A.S. for 10 years now and has lost her other children several times. I have taken my daughters mother to court 4 times now for termination of access because of her own neglect towards my daugther and her behaviour with my daughter. I am recently taking her back to court again because her mothers father has a court order stated he is not to be around any child under the age of 15 has been in the presents of my daughter and the last 3 visits that my daughter has had with her mother, her mother abandoned her with her older sister . My daughters mother always gets supervised access (which she refuses to go)and i need it terminated for the safety of my daughter. Is this possible?
