Question A law firm lost my personal and bank account details...

My head office uses a law firm for routine jobs such as applying for a Personal Licence. Recently, among other documents, I filled in and sent them a Disclosure form plus a bank statement as a prove of address. Soon after they called me and said that they cannot find the form nor my statement, and that I will have to send it again. I was outraged and I refused to do so.
What do I do now? Can I complain about this and to whom?
