Confused involuntary termination of parental righs

I am doing the adoption of my stepson. We know where his birth mom lives but are unable to get her to sign her rights over. I have been a mother to my stepson for almost 7 years. His birth mom has not had anything to do with him for the past 7 years. I am needing to do a petition for involuntary termination of parental rights and I would like to know if anyone knows where I could get a sample copy of a petition for involutary termination of parental rights. I would gratly appreciate it. Also, if anyone knows what it is that I will need to show proof that the birth mom has not had any contact nor has she supported my stepson in anyway. I love my stepson like he was my own and it was him who wanted me to start the adoption.

Check with your local court clerk's office, usually they will either have the forms available or will know where you can find them.

You really should be using an attorney, any type of contested adoption is definately not a DIY project because it's just too important to have something go wrong.

I have a friend that is a lawyer that is helping me through this. She checks everything that I type and if something is wrong she will correct it for me. I am just wanting to try to make sure tha I type right to begin with. I am to start law school in the fall and my friend feels this will be a good experience for me. So she is not charging me anything to help me since I am the one doing all of the work. Our clerk's office here doesn't have anything like that here I have already checked.

Does your courthouse have a law library?
If so, check there.

Or, try Googling your state/county's clerk website or just for your state's family law forms on line.
