Guardianship from mother in canada to family member in Virginia

hello,

I am American Citizen in Canada on PR. My 16 year old decided to go back to Virginia to go to school. He is staying with my cousin. Can i just go to the courthouse here in Ontario and sign an affidavit to give her guardianship so she can enroll him in school and seek medical care in my absence? Without going to court? Can i do this without a lawyer?

My husband says i can go to the court house and fill out an affidavit, have the court notarize it and they will make it a legal document that i can send to my cousin.

Any advise would help a great deal. Thank you
